What to bring:
Tenants – Overdue Rent (Staying in Current Unit)
- Identification (Head of Household only)
- For all household members:
- Name, Date of Birth, and SSN (as applicable)
- Required for all household members over age 18
- Current proof of income (within 60 days): 2 pay stubs, unemployment income, termination letter, etc.
- Proof of current housing: Lease or Tenancy-at-Will agreement
- Proof of housing crisis: Notice to Quit (within 90 days) or Eviction Summons (within 60 days)
- Rental ledger – through current month
- Additional supporting documentation may be requested
Tenants – Moving to a New Unit
- Identification (Head of Household only)
- Name, Date of Birth, and SSN (as applicable) for all household members
- Current proof of income (within 60 days): 2 pay stubs, unemployment income, termination letter, etc.
- Required for all household members over age 18
- Proof of current housing: Lease or Tenancy-at-Will agreement
- Proof of housing crisis:
- Notice to Quit (within 90 days) or Eviction Summons (within 60 days) OR
- Doubled-up letter, Health & Safety letter (from a third party/Dept. of Health), Domestic Violence statement, Advocate support statement
- Landlord contact information: name, phone number, and email address
- Additional supporting documentation may be requested
Homeowners
- Identification (Head of Household only)
- Required for all household members over age 18:
- Name, Date of Birth, and SSN (as applicable) for all household members
- Current proof of income (within 60 days): 2 pay stubs, unemployment income, termination letter, etc.
- Proof of current housing: current mortgage statement
- Proof of housing crisis:
- Mortgage statement showing 3+ months behind90-day Right to Cure notice
- Notice of Intent to Foreclose (within 60 days) Foreclosure Notice of Sale (within 60 days) Proof of housing crisis for property taxes/other homeownership payments:
- Current lien notice from city/town
- Documentation of unpaid taxes or other payments placing applicant at risk of foreclosure
- Additional supporting documentation may be requested
Utility Assistance
- Identification (Head of Household only)
- For all household members
- Name, Date of Birth, and SSN (as applicable)
- Required for all household members over age 18
- Current proof of income (within 60 days): 2 pay stubs, unemployment income, benefit letter, etc.
- Proof of current housing: Lease or Tenancy-at-Will agreement
- Proof of crisis: disconnection notice from utility company
- Additional supporting documentation may be requested
